Note to Pharmacists:

You can quickly pre-register your patients in the JAMP Care™ Patient Support Program (PSP) by providing the following information about your patient via phone, fax, or email: first and last name, date of birth, phone number, name of the treating physician, and name of the medication. Upon receipt of a pre-registration request, our PSP team will promptly contact the physician and the patient to confirm enrollment and proceed with the next steps.

By pre-registering a patient with the PSP, you acknowledge that you have obtained the patient’s explicit consent to allow the PSP team to contact them for the next steps in the formal registration process. Additional consent will be requested from the patient, if necessary, for any subsequent steps or actions related to the PSP. This procedure ensures that the patient’s participation is fully informed and that all necessary authorizations are obtained for their involvement in the PSP.

By contacting the JAMP Care™ program, you acknowledge that: (i) no personal information is collected through this form; (ii) registration will be completed directly with a program representative; (iii) you will be asked to provide personal and health-related information in order to enroll in the JAMP Care™ program, as well as valid proof that you are currently taking one of the eligible medications from JAMP Pharma or BioJAMP®; and (iv) you will be informed of the purposes of data collection as well as your rights, including the right to withdraw your consent at any time. All details will be explained to you when you contact the program.
